Once you’ve filled it in, return the registration form along with your $60 deposit to your Team Manager at training night – Mondays 4pm-6pm.
If this is your first season with the Mill Park Rebelz, we will also require a copy of your Birth Certificate/Passport with the form & payment.
Fee structure (2016) are as follows:
1st child $ 180 ea
2nd child $160 ea
3rd and subsequent children $140 ea
